Before the National Health Service was created in 1948, patients were generally required to pay for their health care. Free treatment was sometimes available from charitable voluntary hospitals. Some local authorities operated hospitals for local ratepayers (under a system originating with the Poor Laws).

The National Health Service Act 1946 (9 & 10 Geo. 6. c. c. 81) came into effect on 5 July 1948 and created the National Health Service in England and Wales thus being the first implementation of the Beveridge model .

The Royal Commission on the National Health Service was a Royal commission set up by the Wilson government in 1975. It was to consider the "best use and management of the financial and manpower resources of the NHS". The Royal Commission reported in June 1979, by which time the government had changed.
