In the late 1930s SR 22 was moved to a new alignment between Hartford City and the Ohio state line. The entire road was paved before the late 1960s. The road east of SR 26 was removed in the 1970s. Between 2014 and 2016, the Indiana Department of Transportation (INDOT) relinquished the road through Kokomo.
The eastern segment begins at an interchange with Interstate 65 (I-65) and heads east passing through the cities of Kokomo, Hartford City, and Portland, before ending at the Ohio border, where it continues east as Ohio State Route 119. The road covers a distance of about 150.1 miles (241.6 km), passing through mostly rural areas.
