Level 15. It would be taxable by your resident state only --and due to no income tax there, it is not taxable by the State. No Ohio taxes need to be paid by you or an Ohio return needing to be filed for this 1099R. Sorry for your loss.

In addition to saying that you are a non-resident, you have to allocate your income to Ohio. A few screens into your Ohio state return, your will see a screen called, Income Allocations.
