On 3 June 1966, a newly built Hawker Siddeley Trident jetliner crashed during a pre-delivery test flight near the village of Felthorpe, Norfolk, England, killing all four crew. The aeroplane had entered a deep stall from which the crew were unable to recover. It was the first loss of a Trident aircraft. [1]

Avianca Flight 03 was a passenger flight from Cartagena, Colombia to Bogotá, Colombia that crashed on January 14, 1966. After takeoff, as the aircraft reached 100 ft (30 m), it stalled and crashed into shallow water. After a 14-month investigation, engine failure was found to be a possible, but unproven, cause.
The captain (35) held an airline pilot's license with instrument ratings on the Douglas DC-3 and Hawker Siddeley HS 748.His records indicated that he was scheduled for a route qualification check on the Manila-Mamburao-San Jose route in September 1965, however because Mamburao Airport was closed for operations at the time, it was redirected to Lubang.
It was the fifth crash in Japan in 1966 and the second one experienced by All Nippon Airways that year, the first being the loss of Flight 60 on February 4. It was also, at the time, the deadliest crash of an NAMC YS-11, and remains the second-deadliest after Toa Domestic Airlines Flight 63 , which crashed in 1971 with 68 deaths.
