She suffered heavy losses while fighting the cloud creature that was ultimately killed in "Obsession." Note that the class of the USS Republic is also uncertain.
The name Scimitar (to my knowledge) never appeared on any suggested list of names. So in the very least the ships we actually saw were Enterprise, Excaliber, Exeter, Lexington, Hood, Potemkin, Constellation and Defiant. That is 8 out of 12 with four remaining unknown or at least specifically identified.

Originally the Enterprise was referred to as "Starship-class", but in some later TOS episodes, the term "Constitution-class" comes along, although mostly in props and background material.
