Excessive crankcase pressure will not only occur due to blow-by gases escaping past the piston rings but can also be introduced when positive pressure from the intake manifold makes its way into the crankcase.
The positive crankcase ventilation (PCV) system utilized a plastic oil separator box that was vented directly to the block itself; the breather hose and PCV valve hoses attached to the box, and connected to the induction system. Later engines featured a PCV system that was molded to the cylinder head valve cover.

Many two-stroke engines use a crankcase-compression design, where a partial vacuum draws the fuel/air mixture into the engine as the piston moves upwards. Then as the piston travels downward, the inlet port is uncovered and the compressed fuel/air mixture is pushed from the crankcase into the combustion chamber.
After having PCV issues with my personal car I have added some information regarding the additional function of PCV valves and the system as a whole in boosted cars. I have also briefly touched on the scavenging effect and how ideally a PCV system would keep the crankcase under slight negative pressure.
