Diagnosis of complications of varicella-zoster, particularly in cases where the disease reactivates after years or decades of latency, is difficult. A rash (shingles) can be present or absent. Symptoms vary, and there is a significant overlap in symptoms with herpes-simplex symptoms. [122]
There is no treatment that modifies the disease course of PHN; therefore, controlling the affected person's symptoms is the main goal of treatment. Medications applied to the skin such as capsaicin or topical anesthetics (e.g., lidocaine) are used for mild pain and can be used in combination with oral medications for moderate to severe pain. [2]
Varicella zoster virus is known to recur (after its initial presentation as chicken pox) as herpes zoster ("shingles"). Shingles, or herpes zoster, is a painful skin rash with blisters that, characteristically, occurs in a stripe limited to just one side of the body. The rash usually heals within 2–5 weeks, but around one in five people experience residual nerve pain for months or years. Shingles is caused by varicella zoster virus (VZV), an alpha-herpesvirus.
On average, Ramsay Hunt syndrome patients begin to see their symptoms resolve between 5 and 12 months post diagnosis and can expect to see continued resolution of symptoms for up to 2 years post diagnosis. Occasionally, patients may experience minor improvements beyond 2 years. The order in which symptoms resolve is highly individual.
Herpes zoster ophthalmicus (HZO), also known as ophthalmic zoster, is shingles involving the eye or the surrounding area. Common signs include a rash of the forehead with swelling of the eyelid .
