Pulsus paradoxus, also paradoxic pulse or paradoxical pulse, is an abnormally large decrease in stroke volume, systolic blood pressure (a drop more than 10 mmHg) and pulse wave amplitude during inspiration. Pulsus paradoxus is not related to pulse rate or heart rate, and it is not a paradoxical rise in systolic pressure.
Though these are the classical findings; all three occur simultaneously in only a minority of patients. [1] Patients presenting with cardiac tamponade may also be evaluated for pulsus paradoxus. Pulsus paradoxus is a phenomenon in which systolic blood pressure drops by 10 mmHg or more during inspiration.

Pulsus paradoxus: A pulsus paradoxus is an exaggerated decrease in systolic blood pressure during inspiration. It can indicate certain medical conditions in which there is reduced cardiac output, such as cardiac tamponade or constrictive pericarditis .
