The goal of One is for the player to discover the identity of the main character John Cain. The only clue available is a barcode on the player-character's neck. At the beginning of the game, John Cain awakens on the floor of an apartment building, with a gun in place of one arm, no memory, and police helicopters shooting missiles through a window.

The game's beginning introduces another Three-Eyed One called Prince Godaru, riding on the ancient tank Gomorrah. Godaru destroys a part of the city and kidnaps Sharaku's friend, Wato Chiyoko. The rest of the game's plot consists of Sharaku clearing several areas in 5 levels to find Chiyoko and Godaru and defeat the enemies in his way.
A total of 50 developers worked on the game. The game's story was written in 6 months and took 15 months to develop. Masamune Shirow assisted in the conceptual stages of the game. Cavia wanted the game to be faithful to the original TV series, but also chose a design that allowed them to enhance the personalities of Motoko Kusanagi and Batou.
