First, the fundamental issue is that COX - for some bizarre reason - DEFAULTS many of its NFL/Football broadcasts to SPANISH as the PRIMARY language. That means if you go to your TV language and COX menu language/SAP and hit "reset" (or default or "English") you will find that your broadcast stays in Spanish.

All channels and all programs are in English except for one football game on local channel being broadcast in spanish. Settings are all in english. Tried reprogramming box with no change. Also on second tv football game is in english.

Pre-season football on channel 7 would start in English and then switch to Spanish and reverted back twice, finally settling on Spanish. I actually kept the recorded game and played it for the Cox representative on the phone so he could hear it switch between languages.
Only during football games that I've seen. Commercials and other TV shows are in English, so I know it is not the SAP setting. It first happened during Thursday Night Football between the 49ers and Cardinals, then it happened for the USC vs. Oregon NCAA football game. On Sunday, 11/3/19 all NFL games on both 11 and 12 were in Spanish.
