Highway 4 enters the city limits of Port Alberni 2 km (1.2 mi) west of the main spur into the city, and 5 km (3 mi) later, exits Port Alberni via a bridge over the Somass River. Tofino was a strong advocate of the Trans-Canada Highway system, and this sign was erected in the hope that Highway 4 would become part of the TCH.

The Nanaimo River is part of the traditional territory of the Snuneymuxw and Stz'uminus First Nations. In total, the Snuneymuxw have six Indian reserves in the Nanaimo area. Highway 19's northern end is located at the Bear Cove ferry terminal, across the bay from Port Hardy. The highway proceeds southwest from the ferry dock for 5 km (3.1 mi) to a junction with the main road to the centre of Port Hardy, then turns southeast, travelling for 16 km (9.9 mi) to Highway 30, and then further east for 20 km (12 mi) to the main road to Port McNeill.
