Swan Quarter–Ocracoke: The newest route in the system, service was begun across the Pamlico Sound between Swan Quarter and Ocracoke in 1977; it links NC 45. This was done to connect residents of Ocracoke to their county seat for Hyde County .
NC 45 starts at the ferry terminal at Ocracoke, connecting with NC 12, it traverses across the Pamlico Sound along the Swan Quarter-Ocracoke Ferry. At Swan Quarter, it continues at a northwesterly direction; merging with several highways along the way, including US 264, NC 99, NC 32, US 64, NC 308, and NC 461.
Swan Quarter is in western Hyde County at latitude 35.405 N and longitude 76.331 W. The elevation is 3 feet (0.91 m) above sea level. It is located on Swanquarter Bay, an inlet of Pamlico Sound. U.S. Route 264 runs along the northern edge of the community, leading east 69 miles (111 km) to Manteo and west 51 miles (82 km) to Washington.

Southbound NC-12 continues from the ferry terminal across Cedar Island to meet with U.S. Route 70 on the mainland, near the community of Sea Level. The Cedar Island ferry terminal provides access to Swan Quarter and Ocracoke Island, both across the Pamlico Sound.
