The term or article title appears in the author position. Use sentence case for multiple-word terms or titles, where you capitalize the first word, the first word after a colon, and proper nouns. The proper in-text citation is ("Plagiarism," 2004) for a paraphrased passage or ("Plagiarism," 2004, para. #) if you directly quote the material.
Italic formatting cannot be part of the actual (stored) title of a page; adding single quotes to a page title will cause those quotes to become part of the URL, rather than affecting its appearance. A title or part of it is made to appear in italics with the use of the DISPLAYTITLE magic word or the {{ Italic title }} template.
I don't see why they should be treated differently from titles of books, or why there should be a distinction between popular song titles and the titles of orchestral works, for example. A quick flick through som emusic mags seems to back me up on this - all titles are generally italicized when quoted within a body of text.
Grandpa was in World War II, not "World War II". As for the italicized case, we don't use quotation marks and italics at the same time; "failing" to also use quotation marks with the (incorrectly) italicized #4 examples doesn't create any ambiguity, and adding them doesn't make the example any clearer, just twice as wrong.
