The Aviation Safety Reporting System, or ASRS, is the US Federal Aviation Administration's (FAA) voluntary confidential reporting system that allows pilots, air traffic controllers, cabin crew, dispatchers, maintenance technicians, ground operations, and UAS operators and drone flyers to confidentially report near misses or close call events in the interest of improving aviation safety.
Form 8110-3 is the only FAA form that any DER may sign under their designated authority. [14] Order 8110.37() provides instructions for completing Form 8110-3, and includes examples of completed Forms. The reports are analyzed to reduce hazards and focus training. [1] Reporting is encouraged by providing the volunteer reporter protection from certificate action. ASAP forms a safety team between the FAA, the certificate holder (airline/operator), employee, and the operator's employee labor organization. [ 2 ]
Reports are generated once an hour or half-hour at most stations, but if conditions change significantly at a staffed location, a report known as a special (SPECI) may be issued. [ 4 ] : 2 [ note 1 ] Some stations make regular reports more frequently, such as Pierce County Airport (ICAO code: KPLU) which issues reports three times per hour. [ 5 ]
The Functional Safety process is focused on identifying functional failure conditions leading to hazards. Functional Hazard Analyses / Assessments are central to determining hazards. FHA is performed early in aircraft design, first as an Aircraft Functional Hazard Analysis (AFHA) and then as a System Functional Hazard Analysis (SFHA).
The FAA's Advisory Circular System is defined in FAA Order 1320.46D. [ 2 ] By writing advisory circulars, the FAA can provide guidance for compliance with airworthiness regulations , pilot certifications, operational standards, training standards, and any other rules within the 14 CFR Aeronautics and Space title, aka 14 CRF or FARs.
