Appendicitis is the most common emergent general surgery related problem to arise during pregnancy. There is a natural elevation in white blood cell count in addition to anatomical changes of the appendix that occur during pregnancy. [14] These findings, in addition to non-specific abdominal symptoms make appendicitis difficult to diagnose.
A physical examination that is positive for abdominal pain categorized as McBurney's point tenderness, Blumberg's sign, Rovsing's sign, Dunphy's sign and psoas sign, could all indicate acute appendicitis and lead to misdiagnosis. [citation needed] However, these physical examination findings are also present in Valentino's Syndrome.
The diagnosis of appendicitis is largely based on the person's signs and symptoms. [12] In cases where the diagnosis is unclear, close observation, medical imaging, and laboratory tests can be helpful. [4] The two most commonly used imaging tests for diagnosing appendicitis are ultrasound and computed tomography (CT scan). [4]

Markle's sign, or jar tenderness, is a clinical sign in which pain in the right lower quadrant of the abdomen is elicited by the heel-drop test (dropping to the heels, from standing on the toes, with a jarring landing). It is found in patients with localised peritonitis due to acute appendicitis. [1]
Signs and symptoms [ edit ] Those with suspected with IAIs usually have acute onset of abdominal pain with signs of local or generalised inflammations such as pain, tenderness (pain on touching), fever, tachycardia (increased heart rate), or tachypnea (increased breathing rate).
Use of the sign has been supported by others. [5] [6] A study published in 2022 found that a positive Blumberg's sign made the diagnosis of acute appendicitis more likely, but that a negative sign did not rule out the diagnosis. In that sense, a positive Blumberg's sign serves as a tool to supplement clinical diagnosis. [7]
