MDSRC reconstructed the road between Frederick and Hagerstown with a 14-foot (4.3 m) wide macadam surface from Frederick to Middletown and from South Mountain to Boonsboro in 1913. The state highway generally parallels I-68 and crosses over the freeway multiple times as both highways pass through mountainous creek valleys and over Martin Mountain and Polish Mountain.
