I don't think you would repeat either. If the alt text is the same as the caption then the caption either becomes self-evident to the sighted person by glancing at it or it's not a replacement for the image. If additional alt text is added, it should be a succinct description that complies with the content policies; see WP:ALT for accessibility guidelines and Help:ALT for examples. Unlike alt text, a caption can contain Wiki markup like ''[[Myriapoda]]''. The caption text is placed underneath the picture.
It also seems like a good idea to work all this out at once.
