Gear two at least at early levels lowers your max hp and possibly stops any kind of automatic hp or staming refill. Then afterwards lowers all your stats temporarily. Now that g2 is maxxed it doesn't really effect him.

For a normal human being it would shorten lifespan, but I think because Luffy is rubber his organs and blood vessel's are as well. Devil's fruits grow and evolve over time (Gears, Zoans, Etc) The accelerated blood would use up more energy thus increase power, but would not shorten life expectancy. 1.
