From its crossing into Lake County over the Kankakee River to its northern terminus, the highway is known as the Casimir Pulaski Memorial Highway. Prior to 2004, the northern terminus of I-65 was only 0.125 miles (0.201 km) north of I-90 (Indiana Toll Road). Traffic going from I-90 to I-65 had to stop at a traffic signal to make a left turn.

The Interstate passes through the Highland Rim and Nashville Basin physiographic regions of Tennessee, and is often used as the dividing line between the eastern and western portions of the former. Of the four states which I-65 runs through, the segment in Tennessee is the shortest, at 121.71 miles (195.87 km) long.
At this intersection, the route becomes two-lane Woodmont Boulevard, and the SR 155 designation ends, although many maps show this segment as being part of SR 155. About 2 ⁄ 3 mile (1.1 km) later the road curves to the southeast, and begins running east-west nearly perfectly straight before reaching an intersection with US 431 (Hillsboro Pike).
