Frontal sinus trephination is a surgical procedure wherein a small opening is made in the floor of the frontal sinus to facilitate drainage of its contents. Drainage of the frontal sinus is done through the floor of frontal sinus above the inner canthus .

The frontonasal duct is a duct through which either frontal sinus drains into the nasal cavity. [2] [3] Each frontal sinus opens into the frontonasal duct by an opening (the opening of frontal sinus or frontal sinus aperture [4]) on the inferomedial part [3] of the floor of the sinus. [4]
The frontonasal duct may or may not drain into the ethmoidal infundibulum - this is determined by the place of attachment of the uncinate process of the ethmoid bone: if the uncinate process is attached to the lateral nasal wall, the frontonasal duct will open directly into the middle nasal meatus; if otherwise, it will drain into the infundibulum.
An enlarged agger nasi cell may encroach the frontal recess area, constricting it and causing mechanical obstruction to frontal sinus drainage. The agger nasi cell may be removed during sinus surgery to open an obstructed frontal sinus drainage pathway.
