I'm on SSP trying to attach Efficiency III to a diamond pick axe with an anvil. Since i don't want to host the image, I'll just draw it out in words: Diamond pick axe ---> Efficiency III book -X-> [empty space] note that the pick has absolutely NO enchantments on it already, and I have also tried this with a diamond sword with Knock back II.
The number of anvil operations you used to create the pick in question will affect the Prior Work Penalty [PWP] associated with the pick and thus the maximum number of times it can be repaired. This limit to the ability to repair items is also a goad to doing 75% repairs, rather than keeping the item 'topped-up'....
Learn how to estimate the number of anvil operations an item has been through by subtracting the repair cost from the displayed cost. See the formula, examples and tips from other players on Minecraft Forum.

I was a bit curious regarding the math with the anvil, so as an exercise, threw together an anvil cost calculator (optimizer). The upgrade costs: 1 Dropper 1 Hopper 2 Redstone torches 3 Comparators 7 Redstone 1 Repeater. 4 Dirt/Anything. IMPORTANT: Put all DIRT inside the dropper BEFORE placing an undamaged anvil on top of the observer OR put 3 dirt inside the dropper and 1 inside the locked hopper AFTER placing an undamaged anvil on top of the observer.
Also, if you are in 1.9 you might want to get Mending (by fishing and trading and from dungeons, temples, etc) so you can keep your gear forever with the only cost being XP, you don't even need anvils or more diamonds to repair them (you could also indefinitely repair items in the anvil if you renamed them in older versions; 1.8 is the only ...
For example, say I have an anvil at x=10 y=50 z=22. You need two command blocks(as of the new snapshot) In the first command block you would use a /testforblock command /testforblock 10 50 22 minecraft:anvil 0. Next, add a comparator next to the command block leading into a second command block with the command /setblock 10 50 22 minecraft:anvil
