The station has two island platforms serving four tracks (compatible with 12-car trains) and one outbound siding track. The outer track side of each platform is closed to permit the passing of express trains. Since the platform is on a slope, the station building is located below the platform. The station is staffed.
Between Kitaōji and Jūjō, trains run beneath the north–south Karasuma Street, hence the name. They link to the other subway line, the Tozai Line, at Karasuma Oike. They also connect to the JR lines at Kyoto Station and the Hankyu Kyoto Line running beneath Shijō Street at the intersection of Shijō Karasuma, Kyoto's central business district.
