Exophoria is a form of heterophoria in which there is a tendency of the eyes to deviate outward. [1] During examination, when the eyes are dissociated, the visual axes will appear to diverge away from one another. [2] The axis deviation in exophoria is usually mild compared with that of exotropia.
Strabismus is more common in children with disorders that affect the brain such as cerebral palsy, Down syndrome, hydrocephalus, and brain tumors. One study has found that children with exotropia are three times more likely to develop a psychiatric disorder in comparison with the general population. [3] [4] [5]

The prism cover test (PCT) is an objective measurement and the gold standard in measuring strabismus, i.e. ocular misalignment, or a deviation of the eye. [1] It is used by ophthalmologists and orthoptists in order to measure the vertical and horizontal deviation and includes both manifest and latent components. [ 1 ]
In studies that used standardized definitions of convergence insufficiency, investigators have reported a prevalence of 4.2–6% in school and clinic settings. The standard definition of convergence insufficiency is exophoria greater at near than at distance, a receded near point of convergence, and reduced convergence amplitudes at near. [4]

From left to right: an accommodative fixation stick, a vertical prism bar, and a horizontal prism bar. The PFR involves placing a prism bar in front of an eye. In a patient with BSV, a natural shift of the eye occurs. When measuring horizontal fusion ranges, base in prisms assess fusional divergence while base out prisms assess fusional ...
[1] Distance Targets (6m) Spotlight – In those patients with amblyopia a spotlight could be used. (Same as near targets). [1] Snellen chart – This is the most commonly used target when assessing a patient's far component of a cover test. [1] Small landmark – Any fixed landmark at eye level, which can be seen through a window. [1]
