Excisional hemorrhoidectomy is a surgical excision of the hemorrhoid used primarily only in severe cases. [1] It is associated with significant postoperative pain and usually requires two to four weeks for recovery. [1] However, the long-term benefit is greater in those with grade III hemorrhoids as compared to rubber band ligation. [45]
Conventional hemorrhoidectomy provides permanent symptomatic relief for most patients, and effectively treats any external component of the hemorrhoids. However, the wounds created by the surgery are usually associated with considerable post-operative pain which necessitates a prolonged recovery period.
Hemorrhoid pain can be managed with a variety of methods. Some recommendations for reducing the pain of hemorrhoids include: cleansing with warm water, hemorrhoid creams, increasing fluids, lying on the site and sitz baths. [17] Medications controlling pain will begin to wear off. This is also true when an epidural or spinal block is given. [5]
Partial internal sphincterotomy, fistulotomy, anal stretch (Lord's operation), hemorrhoidectomy or transanal advancement flaps may all lead to FI postoperatively, with soiling being far more common than solid FI. The "keyhole deformity" refers to scarring within the anal canal and is another cause of mucus leakage and minor incontinence.
Hemorrhoids and constipation in this period are common, and stool softeners are routinely given. [24] If an episiotomy or perineal tear had to be sutured, the use of a donut pillow allows the woman to sit pain-free or at least with reduced pain. Some women feel uterine contractions, called afterpains, during the first few days after delivery.
The signs and symptoms are variable, and in up to 25% of patients there may be no symptoms. [3] The most common signs and symptoms are bleeding, which can vary from minor to severe, rectal prolapse and incomplete evacuation (35%-76% of cases). [4]
Perianal hematoma are caused by the rupture of a small vein that drains blood from the anus. [4] This rupture may be the result of forceful or strained bowel movement, anal sex or caused by heavy lifting, coughing or straining.
