The road was first mapped as an under construction highway from US 25 near Hendersonville north to NC 280 (current NC 146). The first segment opened in 1966 beginning at NC 280 to the US 25 connector near East Flat Rock. In 1969, I-26 was extended north to I-40, and the South Carolina segment was extended to NC 108 near Columbus.
NC 81 begins at the intersection of All Souls Crescent (US 25 north), Hendersonville Road (US 25 south), and Vanderbilt Road in Historic Biltmore Village. It runs northward via Hendersonville Road for 0.3 miles (0.48 km), intersecting US 25A (Lodge Street) along the way.
The highway continues north into Venable, intersecting the eastern terminus of NC 112 (Sardis Road) and then I-26 and US 74 at exit 33 as it reaches the outermost city limits of Asheville. Entering the city, NC 191 interchanges with I-40 (exit 47) and one-half mile (0.80 km) later, with I-26 / I-240 (exit 1B).
