In the legs, bypass grafting is used to treat peripheral vascular disease, acute limb ischemia, aneurysms and trauma.While there are many anatomical arrangements for vascular bypass grafts in the lower extremities depending on the location of the disease, the principle is the same: to restore blood flow to an area without normal flow.
Blood vessel grafts often come from the same leg of the bypass site, which are arteries above and below the knee. These grafts can only be removed and prepared during surgery at the time of the bypass. However, in some cases a leg vein is left in place and connected to the artery (in situ procedure).
Surgical procedures used to treat PAD include bypass grafting, angioplasty, and atherectomy. [10] In 2015, about 155 million people had PAD worldwide. [13] It becomes more common with age. [27] In the developed world, it affects about 5.3% of 45- to 50-year-olds and 18.6% of 85- to 90-year-olds. [7]
For coronary artery disease (ischemic heart disease), coronary artery bypass surgery and percutaneous coronary intervention (coronary balloon angioplasty) are the two primary means of revascularization. [2] When those cannot be done, transmyocardial revascularization or percutaneous myocardial revascularization, done with a laser, may be an option.
These procedures provide reasonable outcomes that are comparable to surgery with the advantage of short hospital stay (day or overnight for most cases) with lower morbidity and mortality rates. Historically performed by interventional radiologists, vascular surgeons have become increasingly proficient with endovascular methods. [ 5 ]
CABG is the best procedure to reduce mortality from severe CAD and improve quality of life. [31] [32]: 153 Operative mortality strongly relates to the patient's age. According to a study by Eagle et al., patients 50–59 years old have an operative mortality rate of 1.8%, while patients older than 80 have a rate of 8.3%. [33]
Surgeries performed may be minimally-invasive endovascular procedures such as a percutaneous coronary intervention (PCI), which involves coronary angioplasty. The angioplasty uses the insertion of a balloon and/or stents to open up the artery. [1] Other surgeries performed are the more invasive bypass surgeries that graft arteries around blockages.
