The subscapularis stabilises the shoulder joint by contributing to the fixation of the proximal humerus during movements of the elbow, wrist, and hand. [1] It is a powerful defense to the front of the shoulder-joint , preventing displacement of the head of the humerus.
The greater tubercle projects as the most lateral structure of the humeral head. Medial to this, in turn, is the lesser tubercle of the humeral head. The subscapularis muscle origin is divided from the remainder of the rotator cuff origins as it is deep to the scapula. The four tendons of these muscles converge to form the rotator cuff tendon.
The coracobrachial bursa is between the subscapularis muscle and the tendon of the coracobrachialis muscle. Between the capsule and the tendon of the subscapularis muscle is the subscapular bursa, this is also known as the subtendinous bursa of the scapularis. The supra-acromial bursa does not normally communicate with the shoulder joint.
The coracoid process acts as an attachment and origin for a large number of muscles (attached muscles not labeled here). The coracoid process is a thick curved process attached by a broad base to the upper part of the neck of the scapula; [2] it runs at first upward and medially; then, becoming smaller, it changes its direction, and projects forward and laterally.
The lesser tubercle provides insertion to subscapularis muscle. Both these tubercles are found in the proximal part of the shaft. The crest of the lesser tubercle forms the medial lip of the bicipital groove and is the site for insertion of teres major and latissimus dorsi muscles.
[1] [2] It passes obliquely downwards and laterally to the front of the greater tubercle of the humerus. [ 1 ] [ 2 ] It forms two bands [ 3 ] : 908 - an anterior one and a posterior one - that insert into the lesser and greater tubercles of the humerus , respectively.
The intrinsic muscles of the scapula include the muscles of the rotator cuff- the subscapularis, infraspinatus, teres minor and supraspinatus. [9] These muscles attach to the surface of the scapula and are responsible for the internal and external rotation of the glenohumeral joint, along with humeral abduction.
The lesser tubercle of the humerus, although smaller, is more prominent than the greater tubercle: it is situated in front, and is directed medially and anteriorly.. The projection of the lesser tubercle is anterior from the junction that is found between the anatomical neck and the shaft of the humerus and easily identified due to the intertubercular sulcus (Bicipital groove).
