Telophase (from Ancient Greek τέλος 'end, result, completion' and φάσις (phásis) 'appearance') is the final stage in both meiosis and mitosis in a eukaryotic cell. During telophase, the effects of prophase and prometaphase (the nucleolus and nuclear membrane disintegrating) are reversed.
Telophase is the last stage of the cell cycle in which a cleavage furrow splits the cells cytoplasm (cytokinesis) and chromatin. This occurs through the synthesis of a new nuclear envelope that forms around the chromatin gathered at each pole.

Role of the centrosome in cell cycle progression. Centrosomes are associated with the nuclear membrane during the prophase stage of the cell cycle. During mitosis, the nuclear membrane breaks down, and the centrosome-nucleated microtubules can interact with the chromosomes to build the mitotic spindle.
