Preoperational anxiety, or preoperative anxiety, is a common reaction experienced by patients who are admitted to a hospital for surgery. [1] It can be described as an unpleasant state of tension or uneasiness that results from a patient's doubts or fears before an operation.
Amnesia is desirable during surgery, so general anaesthesia procedures are designed to induce it for the duration of the operation. Sedatives such as benzodiazepines, which are commonly used for anxiety disorders, can reduce the encoding of new memories, particularly in high doses (for example, prior to surgery in order for a person not to recall the surgery). [2]
In terms of the mechanism of action of benzodiazepines, their similarities are too great to separate them into individual categories such as anxiolytic or hypnotic. For example, a hypnotic administered in low doses produces anxiety-relieving effects, whereas a benzodiazepine marketed as an anti-anxiety drug at higher doses induces sleep. [175]
Ativan (Lorazepam) is commonly prescribed for the treatment of anxiety. It possesses many of the desirable effects of other benzodiazepines with amnesic properties. It is an effective sedative with a medium length half-life and is useful for appointments that are longer than two hours.
Commonly prescribed benzodiazepines in the U.S. are Valium (diazepam) and Ativan (lorazepam), which are used for muscle relaxation and to control anxiety and seizures.
Gabapentin has been prescribed off-label for anxiety despite a lack of research evidence supporting such use, although some studies have indicated that it may relieve anxiety symptoms. The potential anxiolytic effect of tiagabine has been observed in some pre-clinical trials, but its effectiveness has not yet been proved.
