70% if a needle between 1.067 mm (0.0420 in) and 1.651 mm (0.0650 in) is used. [2] On the Birmingham gauge, these correspond to the values 27–24G, 22–20G and 19–16G. [2] PDPH is roughly twice as common in lumbar puncture than spinal anaesthesia, almost certainly due to the atraumatic needles used in spinal anaesthesia. [24]
The needle-through-needle technique involves the introduction of a Tuohy needle (epidural needle) into the epidural space. The standard technique of loss of resistance to injection may be employed. A long fine spinal needle (25G) is then introduced via the lumen of the epidural needle and through the dura mater, into the subarachnoid space.
A spinal needle is inserted between the lumbar vertebrae L3/L4, L4/L5 [10] or L5/S1 [10] and pushed in until there is a "give" as it enters the lumbar cistern wherein the ligamentum flavum is housed. The needle is again pushed until there is a second 'give' that indicates the needle is now past the dura mater.

Spinal anaesthesia (or spinal anesthesia), also called spinal block, subarachnoid block, intradural block and intrathecal block, [1] is a form of neuraxial regional anaesthesia involving the injection of a local anaesthetic or opioid into the subarachnoid space, generally through a fine needle, usually 9 cm (3.5 in) long.
A spinal CSF leak can be caused by one or more meningeal diverticula or CSF-venous fistulas not associated with an epidural leak. [6] [7] [8] A spontaneous spinal cerebrospinal fluid leak may occur sometimes in those with predisposing heritable connective tissue disorders including Marfan syndrome and Ehlers–Danlos syndromes.
