I live in California, and am a big fan of Disneyland attractions, and I think the gruesome death of Deborah Gail Stone on July 8, 1974 should be mentioned. She was working the "America Sings" attraction at Disneyland (in the Carousel of Progress theater), and was crushed between a rotating wall and a stationary one.
