Pramipexole, sold under the brand Mirapex among others, is a medication used to treat Parkinson's disease (PD) and restless legs syndrome (RLS). [8] In Parkinson's disease it may be used alone or together with levodopa. [8] It is taken by mouth. [8] Pramipexole is a dopamine agonist of the non-ergoline class. [8]
Dexpramipexole is the (D)-enantiomer of pramipexole. Enantiopure dexpramipexole has essentially no dopamine agonist activity and shares no other pharmacologic similarity to pramipexole. [ 10 ] In contrast, pramipexole the (S)-enantiomer is a dopamine agonist and is an approved drug (Mirapex) used in Parkinson's disease.
Due to the major adverse effects of ergot-derived dopamine agonists, they are generally not used in modern medicine and have mostly been abandoned in favor of non-ergot agonists such as pramipexole, ropinirole and rotigotine. They do not induce as serious side effects although common side effects are nausea, edema and hypotension.
It can also reduce the side effects caused by selective serotonin reuptake inhibitors, including Parkinsonism syndrome as well as sexual dysfunction and erectile dysfunction caused by either SSRIs [8] or antipsychotics. A 2008 meta-analysis found that ropinirole was less effective than pramipexole in the treatment of restless legs syndrome. [9]
This is a very unusual side effect. Mainly all substances that mimic the effect of dopamine has adverse effects such as severe hypotension and sudden "sleep attacks". The most common side effects include abdominal pain, vomiting, diarrhea, back pain, and heartburn. [4] Berotralstat was approved for medical use in the United States in December 2020, [3] [4] [9] and in the European Union in April 2021. [5]
As with other dopamine agonists (like pramipexole and ropinirole), compulsive behavior like pathological gambling, overeating, excessive shopping, increased libido, sexual and/or other intense urges, may develop. [8] [9] Another rare side effect of piribedil is excessive daytime sleepiness and unintended sleep episodes. [9] [10]
General side effects for rotigotine may include constipation, dyskinesia, nausea, vomiting, dizziness, fatigue, insomnia, somnolence, confusion, and hallucinations. [12] [13] More serious complications can include psychosis and impulse control disorders like hypersexuality, punding, and pathological gambling. [14]
