Sacroiliitis is inflammation within the sacroiliac joint. Sacroiliac joint dysfunction is an outcome of either extra-articular dysfunction or from intraarticular dysfunction. There must be clear evidence of sacroiliitis (at least grade 2 bilaterally or grade 3 unilaterally) on the radiographs to diagnose ankylosing spondylitis.
