On February 15, 2024, President Bongbong Marcos signed Republic Act No. 11978, the “Don Mariano Marcos Memorial State University-South La Union Campus-College of Medicine” which offers a Doctor of Medicine Program, including an Integrated Liberal Arts and Medicine Program, the First medical school in La Union in Agoo. Under Dr. Malto's leadership, the state college became ISO-certified (ISO 9001:2008 - Quality Management System). A mock audit of the QMS consultant was conducted on October 27–28, 2015 and November 5–6, 2015. Internal audit followed on November 18–20, 2015. Stage 1 audit was conducted on February 11–12, 2016.
Established through Presidential Decree No. 1279 issued by President Ferdinand Marcos on January 6, 1976, Mariano Marcos State University was founded as a merger of two state colleges in the province of Ilocos Norte: the Mariano Marcos Memorial College of Science and Technology (MMMCST) in Batac and the Northern Luzon State College (NLSC) in Laoag City.

The curriculum is composed of three programs: Foundation Years (Grade 7-8), Advancement Years (Grade 9-10), and Specialization Years (Grade 11-12). Capping the term of Dr. Bayogan was the college's elevation from Level 1 to a Level 3 state college, using a nationwide leveling criteria for all state universities and colleges (SUC) in the country. DOSCST has the distinction of having the least budget allocation among Level 3 SUCs.
