The AUA recommends HoLEP or Prostectomy for those larger than 80 gm. I'm not sure about PAE. Urolift can have problems also. If you want a HoLEP later, the metal clips can ruin the blades in the morcelator, and another morcelator has to be inserted after the clip is removed manually. Sometimes clips can become a problem (not sure about the ...
Holep surgery on Jan.02,2019. Prostate size was 75 grams, DR. removed 45 grams. Surgery went well. My Dr, leaves cath in longer than most. Cath removed on Jan 09, 6 days later. Today is 10 days after Holep surgery and 4 days after removal of cath. Stats,,, after cath removed.... High frequency to urinate, about every 45 minutes....
3rd Dr does everything but pushed for Holep. 4th Dr at a large medical center suggested Holep but made it clear he would be happy to refer me to another specialist. He’s the one I have a Holep scheduled for June 6. I’ve read there are a lot of pae failures. And not impressed with Rezum and Fla. I’m stressing over any procedure except Urolift.
Ron: My understanding is that 'prostatectomy' is the general name for all three of the alternative prstate surgeries. So you can have TURP which is based on surgical cutting out of the intruding prostate lobes, laser prostatectomy using a laser (NICE now recommends HoLEP), and UroLift prostatectomy.
Anyway after holep they said wait 2 months, did so and it was 0.66 August 2016, tested again October 2016 routine medical 0.66 again. Pre holep prostate volume was 22cc, estimate is 6 cc abated and 6 cc of chips caught in filter basket for cancer testing, leaving 10cc behind. So now it is 10 cc.
It doesn't leave much tissue behind. The prostate is like your nose and ears, it never stops growing. Tissue left behind will grow. TURP and Aquablation leave more tissue behind than holep. This increases the odds of a 2nd procedure later down the line. At 63, if I live long enough, the odds of a 2nd procedure are high, so I'm going for the holep.
Holep - Green Light (PVP) - TURP (button) - At this time I would appreciate your valued thoughts, experiences, and rationale opinions on which procedure you believe has the Best Overall results. Feel free to include negative aspects of each, but please limit them to personal experiences, not third/fourth hand stories. Thank you all in advance.
More tissue removed, (the capsule remains) from Holep means no additional procedure necessary. That is my understanding based on reading and the uro I consulted with on Friday. The Holep is more involved with a longer recovery time vs GL. If I get 5 years with GL, perhaps a new or improved procedure will be available.
My understanding is incontinence is very rare with HoLep. You may have incontinence immediately following the surgery, however it is common for it subside a few weeks post-op. If you have a full HoLep, you will have retrograde ejaculation. If you are fortunate enough to have a median lobe - Holep (only removes the median lobe).
After Holep I experienced a very minor incontinence that resolved within a month. Yes, I was informed about possible complications but gave my consent as quality of life was miserable otherwise. There is a lot of feedback from the US and Russian patients, where Holep seems to be more popular.
