The district consists of the municipalities of Alimodian, Leganes, Leon, New Lucena, Pavia, San Miguel, Santa Barbara and Zarraga. Until 1995, it previously included the former sub-province of Guimaras, which became a separate province in 1992.

San Miguel is 17 kilometres (11 mi) northeast of Iloilo City and 8 kilometres (5.0 mi) from Alimodian. The municipality is bordered by Cabatuan in the north, Santa Barbara and Pavia in the east, Alimodian and Leon in the west, and by Oton in the south. Southeast of San Miguel is Mandurriao district of Iloilo City.
