The cervical canal is generally lined by "endocervical mucosa" which consists of a single layer of mucinous columnar epithelium. However, after menopause, the functional squamocolumnar junction moves into the cervical canal, and hence the distal part of the cervical canal may be lined by stratified squamous epithelium (conforming to a "type 3 transformation zone").
The leading hypotheses for its mechanisms were that it could help keep the cervix closed similarly to the cerclage, as well as change the inclination of the cervical canal so that the pregnancy weight is not directly above the internal os.
Prior to effacement, the cervix is like a long bottleneck, usually about four centimeters in length. Throughout pregnancy, the cervix is tightly closed and protected by a plug of mucus. Effacement is accompanied by cervical dilation. When the cervix effaces, the mucus plug is loosened and passes out of the vagina.
Effacement translates to how 'thin' the cervix is.
