Keentown Road is a 1.8-mile (2.9 km) dirt road that leads into the hamlet of Keentown, a farming community. There are only 3 major junctions on this highway, Florida State Road 37, CR 39, and CR 663. Five miles (8.0 km) east of Parrish is Lake Parrish, an artificial lake. It is home to an FPL power plant and a boat ramp.

The road was added to the state highway system in the 1920s with various designations. The route was originally SR 18 from Palma Sola Bay to Bradenton, SR 63 from US 41 in Bradenton to a point east of Zolfo Springs (at present-day SR 636 ), and SR 32 east of there to Avon Park.

Originally, there was no State Road connection between I-95 and Key Biscayne when the Interstate highway was opened in 1967. The southernmost exit of the expressway was a short "trunk ramp" forming the southwestern end of Southwest 23rd Road at Southwest First Avenue (with the ramps having an at-grade crossing with Florida East Coast Railroad tracks before reaching First Avenue).
