The Indiana Department of Health Division of Vital Records is responsible for maintaining and issuing certified copies of vital records, including birth, death, and fetal death certificates for events that occurred in Indiana.
Death Certificates. Please print State Form 49606, Application for Search of Certified or Non-Certified Copy of a Death Record. The form includes applicable fees, identification requirements, and mailing the address. The cost for the first certificate is $8.00 and $4.00 for each additional copy.
