Paul III's artistic and architectural commissions were numerous and varied. The Venetian artist Titian painted a portrait of the pope in 1543, and in 1546, the well-known portrait of Paul III with his grandsons Cardinal Alessandro Farnese and Ottavio Farnese, Duke of Parma. Both are now in the Capodimonte Museum in Naples.

The work is often compared to Raphael's Portrait of Pope Leo X with Cardinals of 1518–19 and the 1511–12 portrait of Julius II for its colouring and psychological dynamic.
