It is the first highway in Thailand to meet international standards, and the first highway in Thailand to use both asphalt and concrete. It received the name "Thanon Mittraphap" on 20 February 1957. The name literally means "Friendship Road". It is the main road that connects Isan (northeastern Thailand) across the Dong Phaya Yen Range.

The under-construction (as of 2022) Motorway 6, with an access ramp serving Muak Lek, is expected to supplant Mittraphap as the most direct route from Bangkok to Nakhon Ratchasima.
The poor condition of these roads meant that rail transport continued to be an important mode of national transportation even during the road-oriented period from the policy shift after the 1932 revolution.
