Inner thighs are much more sensitive, and presumably have more nerve endings. I know this from personal experience, as I just don't have the same reaction if my wife runs her hand across my outer thigh. So, seriously, I believe it's a combination of effectiveness (flesh/fat vs muscle) and comfort (nerves).

I find it to be very comfortable and have had some excellent results there. My longest lasting sensor was a thigh site, 17 days, and was one of the most accurate ones I've had. I have tried thigh sites that were on the sides, and further down and that did not work as well. Also tried inner thigh but it wasn't very good and felt weird.

If you don't have enough body fat on your thighs, it may be very difficult to perform a subcutaneous injection. On my thigh, I can pinch about a 1/8-1/4 inch and so I have to really pinch and pull the skin and then make a shallow angled injection (I inject byetta) in order to get it subcutaneous. It does not work well.
