The medullary pyramids contain motor fibers that are known as the corticobulbar and corticospinal tracts. The corticospinal tracts are on the anterior surface of the pyramids. These tracts transport motor signals that originated in the precentral gyrus and travelled through the internal capsule to the medulla oblongata and pyramids.
Between the pyramids are the cortical infoldings, called columns of Bertin (Figure 1). In the pediatric patient, it is easier to differentiate the hypoechoic medullar pyramids from the more echogenic peripheral zone of the cortex in the parenchyma rim, as well as the columns of Bertin (Figure 2). The medullary interstitium is the tissue surrounding the loop of Henle in the medulla. It functions in renal water reabsorption by building up a high hypertonicity , which draws water out of the thin descending limb of the loop of Henle and the collecting duct system .

The renal columns, Bertin columns, or columns of Bertin, a.k.a. columns of Bertini are extensions of the renal cortex in between the renal pyramids. They allow the cortex to be better anchored. (Cortical extensions into the medullary space.) Each column consists of lines of blood vessels and urinary tubes and a fibrous material.
The corticobulbar (or corticonuclear) tract is a two-neuron white matter motor pathway connecting the motor cortex in the cerebral cortex to the medullary pyramids, which are part of the brainstem's medulla oblongata (also called "bulbar") region, and are primarily involved in carrying the motor function of the non-oculomotor cranial nerves, like muscles of the face, head and neck.
