Attacks of skin redness and burning sensation or pain in one or both external ears are the only common symptoms. [1] Pain is often most pronounced at the ear lobe, and sometimes radiates to the jawbone and cheek. [1] The pain is normally mild, but has occasionally been described as severe. [1]
Similar brief shocks can be experienced when any other nerve is tweaked (e.g. a pinched neck nerve may cause a brief shock-like paresthesia toward the scalp). In the older age group, spinal column irregularities may tweak the spinal cord briefly when the head or back is turned, flexed, or extended into brief uncommon positions (Lhermitte's sign).
It may or may not be accompanied by other symptoms such as fever, sensation of the world spinning, ear itchiness, or a sense of fullness in the ear. The pain may or may not worsen with chewing. The pain is usually constant, described as aching or burning, and often affects both sides of the face (this is almost never the case in patients with trigeminal neuralgia). The pain frequently involves areas of the head, face, and neck that are outside the sensory territories that are supplied by the trigeminal nerve.
These symptoms tend to last for a short period of time. Long-term symptoms include muffled and distorted hearing, ear pain, ear numbness, and burning sensations in and around the ear down to the neck. Burning sensations can extend to the cheek and jaw area. [5]
Dysesthesia is distinct in that it can, but not necessarily, refer to spontaneous sensations in the absence of stimuli. In the case of an evoked dysesthetic sensation, such as by the touch of clothing, the sensation is characterized not simply by an exaggeration of the feeling, but rather by a completely inappropriate sensation such as burning.
Early symptoms include intense pain in one ear, the jaw on one side or the neck on one side which may precede the acute facial paralysis by a week or more. Acute symptoms include: acute facial nerve paralysis; pain in the ear, jaw and/or neck; taste loss in the front two-thirds of the tongue; dry mouth and eyes
