Rheumatoid disease of the spine is a morbid consequence of untreated longstanding severe cervical spinal rheumatoid arthritis (RA)–an inflammatory autoimmune disease that attacks the ligaments, joints, and bones of the neck.
Ankylosing spondylitis (AS) is a type of arthritis from the disease spectrum of axial spondyloarthritis. [5] It is characterized by long-term inflammation of the joints of the spine, typically where the spine joins the pelvis. [2]
A rare but severe complication of this disease is vertebrobasilar insufficiency. [7] This is a result of the vertebral artery becoming occluded as it passes up in the transverse foramen. The spinal joints become stiff in cervical spondylosis. Thus the chondrocytes which maintain the disc become deprived of nutrition and die.
Inflammatory back pain associated with ankylosing spondylitis usually starts slowly, has a dull feel to it, and spreads into the gluteal areas. Back pain has a nocturnal component, gets better with movement, and is worse in the morning. Axial arthritis may begin in the sacroiliac joints and work its way up to the cervical spine over time.
