On 12 March 2015, set A19 was driven more than 700 m (2,300 ft) in the wrong direction at Mount Druitt. At the time of the incident, a Pacific National freight train was travelling towards the train on the same line. The cause of the incident was likely attributed to driver fatigue, and prompted a review of train crew rostering policies.
