So I think what a perihilar infiltrate would mean is that there was/is a substance denser than air in the perihilar region of your right lung. This is my best explanation of what this means, but it would be best to check with your Doctor to be sure, and find out if there is anything you need to do to treat it.

Hi @stephers911, deep breath and one step at a time.You'll notice that I moved your message to this existing discussion where @oakhillbull @bcsurvivor @trixie1313 asked similar questions about the meaning of hypermetabolism activity showing on a PET scan.
I collected my 4 chest x-rays on a cd and went home to view them. I was shocked by the results, I could see the right lung had a partial collapse in my 2019 x-ray. In my 2023 x-ray you could see the change(i could tell in my total volume from PFT test) On 2020 xray report stated perihilar peribronchial thickening unchanged from 2019.
They removed a 3.1cm mass from my upper right lung (stage 1b not 1a because of size). However, the measurements on CT scans refer to ground glass, semi solid and solid parts so it is difficult to compare to your 5mm.
