The Public Health Service Commendation Medal is a decoration of the United States Public Health Service presented to members of the United States Public Health Service Commissioned Officer Corps and to members of any Uniformed Services of the United States who has exhibited a level of proficiency and dedication distinctly greater than that expected of an officer.
Personnel may wear up to three badges above the ribbons or pocket flap on dress uniforms, or in a similar location for uniforms without pockets. Personnel may only wear one combat or special skill badge from either group 1 or group 2 above the ribbons. Soldiers may wear up to three badges from groups 3 and 4 above the ribbons.

The medal is silver, circular and 36 mm in diameter. The obverse shows a left facing crowned portrait of King George VI, surrounded by the inscription "GEORGIVS VI D: G: BR: OMN: REX ET INDIAE IMP:" The reverse bears the inscription "THE KING'S / MEDAL FOR / COURAGE / IN THE CAUSE OF / FREEDOM" in five lines, with the word courage in larger font than other words.
The medal is 1 3/8 inches in diameter, made of gold plated red brass.The obverse depicts a triskelion superimposed over a graticule surrounded by a laurel wreath. The reverse bears the words, in relief, DEPARTMENT OF TRANSPORTATION arched above with the incuse inscription OUTSTANDING ACHIEVEMENT on a scroll below.
