The Texas Medicaid & Healthcare Partnership (TMHP) LTC team supports the LTC provider community in submitting claims through the Claims Management System. TMHP also supports providers as they submit forms via the LTC Online Portal.
If you want to help provide health care to low-income women, families, seniors, and kids, you can enroll as a Texas Medicaid provider through the Provider Enrollment and Management System (PEMS). Client Eligibility Lookup. For current eligibility status, please enter 2 of the following 4 data elements for the Client: Patient Control Number. Date of Birth. Social Security Number. Last Name.
