Cusack was born in Evanston, Illinois into an Irish Catholic family. His parents are writer-actor-producer and documentary filmmaker Richard J. "Dick" Cusack (1925–2003), originally from New York City, [2] [3] [4] and Ann Paula "Nancy" Cusack (née Carolan; 1929–2022), [5] originally from Massachusetts, a former mathematics teacher and political activist.
Cusack and his wife, Ann Paula "Nancy" (née Carolan; 1929–2022), [2] [7] had five children: Ann Cusack, Joan Cusack, Bill Cusack, John Cusack and Susie Cusack, all of whom followed him into the acting profession. [5] Circa 1963–1966, the Cusack family moved from New York City to Evanston, Illinois, where the five children grew up.
Cusack died on 20 September 2023, at the age of 27 [4] by suicide. [11] At the time of her death she was the longest-serving player in the Sheffield United women's squad. [9] On 24 September, a minute's silence was held for Cusack before Sheffield United's Premier League game against Newcastle United.

Cusack died on Morning Star Avenue, Dublin at 5 pm on Wednesday 28 November 1906 from a final heart attack at the age of 59. [1] Shortly before this in January 1906, Cusack's son John suddenly died at age 27, having an impact on him. Cusack and his wife are buried together in Glasnevin cemetery, Co Dublin along with 3 of their children. [12]
